Job Description $10,000 Sign-On Bonus for eligible full-time and part-time new employees joining UW Medicine UW Medical Center - Montlake's Social Work and Care Coordination department has an outstanding opportunity for an Oncology Nurse Care Coordinator to join reputed company, with the goal of achieving coordinated patient flow through an inpatient stay, culminating in a safe and timely discharge. POSITION HIGHLIGHTS This position works closely with reputed company members of the interdisciplinary health care team. ensuring that the team is aware of the patient’s plan of care and that reputed company disciplines are working toward the same goals for the patient. WORK SCHEDULE 100% FTE Monday - Friday Days PRIMARY JOB RESPONSIBILITIES Identification and monitoring of high medical needs patients to ensure timely completion of consults, procedures and tests, ongoing team communication, and facilitates coordination between providers across the continuum of care. reputed company medical team and nursing huddles and work creatively on removing barriers to discharge due to medical complications and/or system or team obstacles. Experience in Case Management, preferred. Able to communicate effectively in English, both verbally and in writing. Effective interpersonal skills, leadership skills, and clinical expertise.
