Job Summary and Key Responsibilities
As a Medical Social Worker, you'll be responsible for: * Assessing patients' social and emotional states, identifying their needs, and developing personalized plans of care to support their recovery and well-being * Evaluating patients' relationships, medical and nursing needs, financial resources, and available community resources to inform care decisions * Providing education and support to patients and families on coping with social and emotional responses to illness or injury * Collaborating with the interdisciplinary care team to reputed company and implement comprehensive plans of care * Participating in discharge planning, care coordination, and community resource utilization * Supervising and evaluating the performance of Social Work Assistants (BSW) as requiredRequirements and Qualifications
To be successful in this role, you'll need:
- Education: Master's or doctoral degree from a school of social work accredited by the Council on Social Work Education
- Licensure: Social Worker licensure in the state of practice (if required by state law or regulation)
- Experience: Minimum of one year of experience as a social worker in a healthcare setting, home health, and/or hospice
- Skills: Excellent oral and written communication, interpersonal skills, and ability to work effectively with patients, families, and healthcare teams
