Job Summary

The Care Coordinator - Social Work performs various direct and indirect patient care activities, working closely with patients and families who are members of the La Rabida Medical Home Program. This role involves providing support for integrated care teams, maintaining external relationships with referring organizations, payers, vendors, and regulatory entities, and coordinating day-to-day care coordination and social work activities. The ideal candidate will possess excellent communication and interpersonal skills, demonstrated knowledge of service delivery systems and resources for children and families, and the ability to reputed company and implement plans of care.

Key Responsibilities

• Assess psychosocial needs and utilize interventions to enhance patient and family functioning and emotional well-being.
• Provide telephonic or in-person/in-clinic crisis management as needed.

Requirements and Qualifications

Essential Qualifications:

• Master's degree in Social Work required.
• 3 years of work experience with children and families required.
• Excellent communication and interpersonal skills.
• Demonstrated knowledge of service delivery systems and resources for children and families.
