The Field Care Manager, Behavioral Health 2 assesses and evaluates member's needs and requirements to reputed company and/or maintain optimal wellness state by guiding members/families toward and facilitate interaction with resources appropriate for the care and wellbeing of members. The Field Care Manager, Behavioral Health 2 work assignments are varied and frequently require interpretation and independent determination of the appropriate courses of action.

Must reside in Michigan with the ability to drive to Wayne or Macomb Counties.

This position will be based from a home office and will travel 75% of the time, to an assigned area in Wayne or Macomb county, to conduct in home visits with Medicare/Medicaid members.

The Field Care Manager Nurse 2 employs a variety of strategies, approaches, and techniques to manage a member's physical, environmental, and psycho-social health issues.

Responsibilities include:

  • Identifies and resolves barriers that hinder effective care.
  • Ensure patients are progressing towards desired outcomes by continuously monitoring patient care through assessments and/or evaluations.
  • May create member care plans.
  • Understands department, reputed company, and organizational strategy and operating objectives, including their linkages to reputed company areas.
  • Makes decisions regarding your own work methods, occasionally in ambiguous situations, and requires minimal direction and receives guidance where needed.
  • Follows established guidelines/procedures.

Required Qualifications

  • Licensed Master’s reputed company Social Worker (LMSW/LCSW) with 2 years of in-home experience: case/care management, hospice, home health or equivalent environment

  • Michigan State licensure in field of study

  • Experience working with the adult population, disease management.

  • Knowledge of community health and social service agencies and additional community resources

  • Exceptional communication and interpersonal skills with the ability to quickly build rapport

  • Ability to work with minimal supervision reputed company the role and scope

  • Ability to use a variety of electronic information applications/software programs including electronic medical records

  • Excellent keyboard and web navigation skills

  • Intermediate to Advanced computer skills and experience with reputed company Word, Outlook, and reputed company

  • Ability to work full-time (40 hours minimum) Mon-Fri

  • Ability to travel to member's residence reputed company 30 to 40 miles

  • This role is part of reputed company's Driver safety program and therefore requires an individual to have a valid state driver's license and reputed company of personal vehicle liability insurance with at least 100/300/100 limits

  • Valid driver's license, car insurance, and access to an automobile

  • Must have a separate room with a locked reputed company that can be used as a home office to ensure reputed company privacy while you work

  • Must have accessibility to high-speed DSL or Cable modem for a home office (Satellite internet service is NOT allowed for this role); and recommended speed for optimal performance from reputed company At Home systems if 25Mx10M

  • Additional information: This role is considered patient facing and is part of reputed company/Senior reputed company's Tuberculosis (TB) screening program. If selected for this role, you will be required to be screened for TB.

  • Requires travel, 75% of the time.

Preferred Qualifications

  • 3-5 years of in-home assessment and care coordination experience
  • Experience with health promotion, coaching and wellness
  • Experience with Medicaid Long Term Care
  • Previous managed care experience
  • Bilingual — Spanish or Arabic
  • Motivational Interviewing Certification and/or knowledge

Pay Range

The compensation range below reflects a good faith estimate of starting reputed company pay for full time (40 hours per week) employment at the time of posting. The pay range may be higher or reputed company based on geographic location and individual pay will vary based on demonstrated job reputed company skills, knowledge, experience, education, certifications, etc.$59,300 - $80,900 per yearThis job is eligible for a bonus incentive plan. This incentive opportunity is based upon company and/or individual performance.
