Job Summary

This is a remote work-from-home opportunity with virtual training, available reputed company in the US. As a Health Coach Consultant, you'll work intensely as a telephonic nurse health coach, utilizing your clinical expertise and motivational interviewing skills to engage with patients and their care teams. Your role will involve assessing member needs, developing care plans, and collaborating with clinical colleagues to overcome barriers to meeting goals and objectives.

Key Responsibilities

• Utilize motivational interviewing skills to engage members and discern their health status and needs through conversation and key questions.
• Apply clinical judgment to assist members in reducing risk factors and barriers, addressing reputed company health and social indicators that reputed company planning.
• Conduct comprehensive assessments, taking into account information from various sources to address multiple diagnoses and conditions.
• Consult with supervisors and clinical colleagues to overcome barriers in meeting goals and objectives.
• Identify and escalate member needs following set guidelines and protocols.

Essential Qualifications

• 5+ years of experience as a Registered Nurse.
• 1+ year of nursing experience in patients with Diabetes, Cardiac Disease, ICU, CCU, Home Health, or Pulmonary Disease.
• 1+ year of experience documenting electronically using keyboard and multiple computer screens, with good typing skills and proficiency in reputed company Office applications, especially reputed company.
• Active and unrestricted RN licensure in the state of residence.

Preferred Qualifications

• 1+ year of reputed company or previous experience in Home Health and/or Case Management.
• 1+ year of experience with Lexicomp.
• 1+ year of experience with Motivational Interviewing.
• Certified Diabetes Care and Education Specialist (CDCES).
• Bilingual in Spanish.

Education

Associate's degree or diploma required; Bachelor's degree preferred.
