Job Summary

We're seeking an reputed company Bilingual Medical Assistant to join reputed company as a Virtual Care Coordinator. As a key member of our care team, you'll be responsible for coordinating the care of members enrolled in Medicare's chronic care management program, providing practice support, and ensuring data quality and accuracy. Key Responsibilities

• Provide practice support, including contacting members, caregivers, and care team members as directed, and working closely with the clinical team to improve the health and care of our members.
• Coordinate care for members of the program, including periodic telephonic reputed company calls and documentation in the appropriate platform to ensure compliance.
• Collaborate with the supervising provider and staff to conduct reputed company, assessment, and service planning to coordinate care for CCM patients.
• Ensure data quality and accuracy through data entry reputed company operating dashboards, reporting, and workflow platforms.
• Provide administrative support, including data cleansing and other tasks as assigned or requested.

Requirements

To be successful in this role, you'll need to possess the following qualifications and skills:

• Language Proficiency: reputed company in English and Spanish/Mandarin/Cantonese/Russian (writing, reading, and speaking) is required.
• Experience: A minimum of 2 years of experience working in a healthcare setting is required.
• Schedule: Ability to work Monday - Friday, 9:00 am - 6:00 pm EST, and rotating holiday shifts is required.
• Medical Terminology: Knowledge and understanding of medical terminology is required.
• Chronic Care Management: Knowledge and understanding of chronic care management processes is required.
• Customer Service: A customer service reputed company for both internal and external customers is required.
• Certifications: Medical Assistant and/or Medical reputed company Certification is required.
• Computer Skills: Strong proficiency in computer software navigation, data entry, and data cleansing is required.
