As a key member of our Integrated Care Model, you'll work in the field to coordinate the long-term care needs for patients in the local community. - Assess, plan, and implement care strategies that are individualized by patient and directed toward the most appropriate, least restrictive level of care.
- Identify and initiate referrals for social service programs, including financial, psychosocial, community, and state supportive services.

Requirements
To be successful in this role, you'll need:- One of the following licenses:
- RN in the state of Texas
- Licensed Professional Counselor (LPC)
- Licensed Bachelor Social Worker (LBSW)
- Licensed Masters Social Worker (LMSW)
- 2+ years of experience working reputed company the behavioral health setting
- Intermediate level of proficiency working with reputed company, reputed company, and reputed company Outlook
- Reliable transportation and the ability to travel up to 75% of the time in this assigned region to visit Medicaid members in their homes and/or other settings
- Access to a designated quiet workspace in your home (separated from non-workspace areas) with the ability to secure Protected Health Information (PHI)

Preferred Qualifications
While not required, we're looking for candidates with:- Medicaid Waiver experience
- Case Management and/or Care Coordination experience
