About the Role

Key Responsibilities

• Assess patients' social and emotional states, identifying needs and developing plans to address them.
• Evaluate the relationships between patients' medical and nursing needs, financial resources, and available community resources.
• Provide guidance and support to patients and families, helping them access community resources and services that facilitate recovery and well-being.
• Collaborate with physicians and other healthcare team members to understand the social and emotional factors influencing patients' health and reputed company effective plans of care.
• Participate in care coordination activities, acting as a resource to other team members and contributing to the identification and resolution of patient needs.
• Supervise and instruct Social Work Assistants (BSW) to ensure compliance with company, government, and professional standards.
• Maintain accurate and timely documentation, including case conferences, patient/physician community contacts, visit reports, and reputed company notes.
• Participate in discharge planning, ensuring a smooth transition for patients as they move through the healthcare continuum.

Essential Qualifications

• Master's or doctoral degree from a school of social work accredited by the Council on Social Work Education.

Compensation and Perks

We're offering a competitive compensation range of $52,500 - $72,225 per year for this per diem role, depending on your experience and qualifications. As a per diem employee, you'll have the flexibility to choose your schedule and work hours, allowing you to balance your work and personal life.
