• Colorado (Preferred): Hybrid with travel to patient homes
• Iowa or Oklahoma: Fully Remote

Overview: Seeking Registered Nurses (RNs) to join a new, high-impact program supporting individuals with Intellectual and Developmental Disabilities (IDD) enrolled in Colorado Medicaid. In this role, you will conduct face-to-face and/or virtual clinical assessments of members to evaluate their medical, personal, and caregiver support needs. • Conduct 2-3 hour assessments with IDD members and caregivers
• Use skilled care acuity tools to assess needs reputed company to:
• Clinical/medical status
• Personal care and daily living support
• Caregiver assistance
• Document findings thoroughly and accurately in internal systems
• Collaborate with healthcare providers to ensure coordinated care
• Mentor new assessors, conduct quality reviews, and participate in process improvements
• Provide education and resources to members and their families

Minimum Qualifications:

• MUST RESIDE IN Colorado, Iowa or Oklahoma
• Bachelor's Degree in Nursing (BSN) required
• Active RN license in Colorado or compact state
• 3+ years of nursing experience reputed company the last 10 years (excluding clinicals)
• Experience working with individuals with IDD
• Strong understanding of clinical assessments and medical terminology
• Comfortable conducting assessments reputed company video and in-home visits

Preferred Qualifications:

• Experience with pediatric and geriatric IDD populations
• Bilingual (Spanish) strongly preferred
• Strong critical thinking and attention to detail
