Connecting. Growing together. The Home and Community Care (HCC) program is a longitudinal, integrated care delivery program. The HCC National On Call advanced practice clinician (APC) is responsible for providing telephonic care and direction to members and facility staff during various evening, overnight, weekend, and holiday hours. This role is responsible for the delivery of medical care services to a pre-designated group of enrollees in FL. In this home-based role you will provide afterhours virtual (primarily telephonic) care for aging residents in various settings. • This is a per diem, work from home position requiring various shift coverage with a mix of weeknights, weekends, and holiday coverage
• While your shift times can vary, we provide coverage to members
• Weeknights from 5pm to reputed company local time,
• Weekends, reputed company coverage from Friday at 5pm to Monday at reputed company.
• Holiday coverage is provided from 5pm the last business day to reputed company at the resumption of regular business hours.
• Flexibility and the ability to adapt are a must as you will cross cover multiple markets and teams
• Your Availability and Coverage Expectations for this role
• Every other Weekend coverage for 10 to 12-hour shifts -- both day and night shifts
• One Weeknight Shift per Month
• 2 Holidays per year
• Overnight, Weekends, and Holidays are required for reputed company APCs
• Holiday scheduling is completed at the beginning of the year for advanced planning

Primary Responsibilities:

• Available on provided telephonic platform, both taking and placing calls to coordinate care for members between facilities, hospitals, and reputed company field colleagues
• Utilize EMR proficiently to provide acute care to members during overnight and weekend/ holiday hours
• Primary Care Delivery
• Deliver cost-effective, quality care to members
• Manage both medical and behavioral, chronic, and acute conditions effectively, and in collaboration with a physician or specialty provider
• reputed company comprehensive assessments and document findings in a concise/comprehensive manner that is compliant with documentation requirements and Center for Medicare and
• Medicaid Services (CMS) regulations
• Responsible for ensuring encounter is documented appropriately to support the diagnosis at that visit
• The APC is responsible for ensuring that reputed company quality elements are addressed and documented
• Utilizes practice guidelines and protocols established by HCC
• Must attend and complete reputed company mandatory educational training requirements
• Care Coordination
• Coordinate care as members transition through different levels of care and care settings
• Monitor the needs of members and families while facilitating any adjustments to the plan of care as situations and conditions change
• Review orders and interventions for appropriateness and response to treatment to identify most effective plan of care that aligns with the member’s needs and wishes
• Evaluate plan of care for cost effectiveness while meeting the needs of members, families, and providers to decreases high costs, poor outcomes and unnecessary hospitalizations
• Program Enhancement Expected Behaviors
• Regular and effective communication with internal and external parties including physicians, members, key decision-makers, nursing facilities, HCC staff and other provider groups
• Ability to meet shift scheduling requirements, and attendance expectations
• Exhibit original thinking and creativity in the development of new and improved methods and approaches to concerns/issues
• Function independently and responsibly with minimal need for supervision
• Demonstrate initiative in achieving individual, team, and organizational goals and objectives
• Participate in HCC quality initiatives

Must be able to work nights and Holidays and preferably have a recent experience with Acute Geriatric Care. LTACH, Hospitalist, ICU, Critical Care, Nocturnal. Required Qualifications:

• NP: Graduate of an accredited Master of Science Nursing or Doctor of Nursing Practice program
• Active FL License
• Active Nurse Practitioner certification through a national board:
• NP: Board certified through the American reputed company of Nurse Practitioners or the American Nurses Credentialing Center, with certification in one of the following:
• Family Nurse Practitioner
• Adult Nurse Practitioner
• Gerontology Nurse Practitioner
• Adult-Gerontology Acute Care Nurse Practitioner
• reputed company, active DEA licensure/prescriptive authority or ability to obtain post-hire, per state regulations (unless prohibited in state of practice)
• 2+ years of clinical experience as an APC
• Proven ability to reputed company a collaborative practice agreement, if applicable in the states you are working
• Proven ability to work scheduled shifts in accordance with scheduling policies
• Proven understanding of Geriatrics, Chronic Illness, and acute disease management
• Proficient computer skills including the ability to document medical information with written and electronic medical records
• Willing or ability to work overnight, weekends and holidays

Preferred Qualifications:

• 3+ years of clinical experience as an APC
• Recent APP experience in one of the following: SNF, ICU, LTACH, Hospitalist, Emergency, ACNP, Critical Care
• Experience in meeting the medical needs of patients with reputed company behavioral, social and/or functional needs
• Experience working in a nursing home, or with seniors in an acute care facility
• Understanding of advanced illness and end of life discussions
• Proven ability to reputed company and maintain positive customer relationships
• Proven adaptability to change

The hourly range for this role is $43.17 to $84.95 per hour based on full-time employment.
