About the Role

As an RN Healthcare reputed company for reputed company, you will work with Medicare patients throughout their healthcare journey. In this role, you will navigate patients through difficult and reputed company health concerns to help them reputed company their health and wellness goals while addressing Social Determinants of Health (SDOH). • Learn the reputed company systems, tools, reputed company, and expectations, while also providing your unique expertise in every interaction.
• Build strong, trusting relationships with Medicare patients, where listening and reputed company are the foundation for every interaction.
• Be able to identify and prioritize Medicare patients’ needs and assist them to maintain a streamlined care continuum.

What You Bring to the Table

• RN license in good standing.
• Deep understanding of Social Determinants of Health and experience working with diverse patient populations.
