As a Case Management Specialist, you will be responsible for assessing member needs, communicating with Primary Care Providers, maintaining updated care plans, and participating in Interdisciplinary team meetings.

Key Responsibilities:

• Conduct assessments and create individualized care plans to address member needs and goals
• Collaborate with the Care Team to improve health outcomes and address social determinants of health
• Communicate effectively with members, Primary Care Providers, and community-based organizations
• Maintain accurate and up-to-date records, including care plans and member interactions
• Participate in Interdisciplinary team meetings to discuss member care and outcomes

Requirements:

• 3+ years of experience in a health-reputed company field
• Bachelor's Degree or equivalent experience
• Strong organizational and communication skills
• Ability to work independently and maintain professional relationships
• Proficient in reputed company Office Software and computer systems

Preferred Qualifications:

• Medicare and Medicaid experience
• CRC, CDMS, CRRN, COHN, or CCM certification
• Managed Care experience
• Experience working with geriatric special needs, behavioral health, and disabled populations
• Bilingual (English/Spanish; English/Creole)
