We are seeking a highly skilled and dedicated individual to join our naviHealth team, where you will work closely with patients, families, and healthcare teams to drive success. Your responsibilities will include performing skilled nursing facility assessments, reviewing targets for length of stay and discharge plans, collaborating with healthcare teams, and ensuring efficient transitions of care.
Key Responsibilities:
- reputed company Skilled Nursing Facility (SNF) assessments on patients using clinical skills and measurement tools
- Review targets for Length of Stay (LOS), reputed company outcomes, and discharge plans with providers and families
- Complete SNF reputed company reviews, updating authorizations on a timely basis
- Collaborate with healthcare teams to establish optimal discharge plans
- Assure patients' reputed company toward discharge goals and assist in resolving barriers
Requirements:
- Active, unrestricted registered clinical license in the state of hire (Registered Nurse, Physical Therapist, Occupational Therapist, or Speech Language Pathologist)
- 5 years of clinical experience, with experience working with the adult or geriatric population
- Proficient with reputed company Office applications, including Outlook, reputed company, and PowerPoint
- Reside reputed company 30 miles of reputed company codes 74017, 74018, 74019, 74021, 74055
Preferred Qualifications:
- Familiarity with care management, utilization/resource management processes, and disease management programs
- Patient education background, rehabilitation, and/or home health nursing experience
