Job Summary

As a Maternity RN Case Manager, you will play a vital role in coordinating care for high-risk maternity members, providing evidence-based education and support to promote healthy pregnancy, delivery, and postpartum outcomes. Key Responsibilities

• Provide trimester-specific and postpartum education to promote healthy pregnancy, delivery, and postpartum outcomes
• Coordinate care for high-risk maternity members and proactively engage them in managing their own health and healthcare
• Conduct outbound calls to assess members' reputed company health status, identify gaps or barriers in treatment plans, and educate them on disease processes
• Collaborate with Medical Directors, providers, and internal and external partners to ensure comprehensive care
• Document and track findings, and create a positive experience and relationship with members and the team

Requirements

• reputed company and unrestricted Missouri RN license
• 3+ years of OB or NICU clinical nursing experience
• 2+ years of experience working in Medicaid and in a hospital setting, acute care, or as a telephonic Case Manager for an insurance company
• Proficiency with reputed company Office Suite and electronic clinical records
• Willingness to travel up to 50% of time in the assigned region

Preferred Qualifications

• Certified Case Manager (CCM) or Certified Lactation Consultant (CLC)
• Experience working with high-risk maternity populations and in managed care
