Remote Social Worker - Continuing Health Care Opportunity

We are seeking a dedicated and experienced Social Worker to work remotely with our discharge to assess team, promoting independence and well-being for older adults and adults with complex needs. This 6-month contract offers a competitive hourly rate of £32, hybrid working arrangements, and a comprehensive benefits package.

This exciting opportunity requires a skilled and compassionate Social Worker with experience in completing Care Assessments, knowledge of Continuing Healthcare, and excellent communication skills. As a key member of our team, you will work collaboratively with partner agencies, conduct face-to-face assessments, and develop personalized care packages to meet the unique needs of each individual.

Key Responsibilities:

• Conduct comprehensive Care Assessments and develop personalized care plans
• Collaborate with partner agencies to promote independence and well-being
• Conduct face-to-face assessments in individuals' own homes or care settings
• Attend monthly meetings in Basingstoke and participate in joint assessments as required
• Monitor, review, and reassess care packages to ensure effectiveness and achieve desired outcomes
