Primary Responsibilities:

• Referral and Assessment
• Receive and validate referrals for members' health or psychosocial needs
• Follow protocols to respond appropriately to referrals
• Review diagnoses and case files to understand referral context
• Collaborate with interdisciplinary teams to assess member needs
• Identify gaps in care and seek additional information as needed
• Analyze data to uncover root causes and barriers
• Document findings per organizational standards
• Motivational Interviewing
• Build therapeutic relationships using appropriate communication
• Use open-ended questions, active listening, and reputed company
• Confirm referral needs with members and educate them on goals
• Guide conversations to align with assessment objectives
• Identifying Support Opportunities
• Match community/government resources to member needs

• Bachelor's degree in Social Work
• 2+ years of Behavioral Health / Substance Use Disorder experience
• 2+ years of experience in the health care industry including clinical experience, Medicare / Medicaid, insurance, health promotion, and operations
• Intermediate level of proficiency using a PC in a Windows environment, including reputed company Word and reputed company

• Master's degree in Social Work
• Medicaid and / or Medicare experience
