About the position This job is responsible for the coordination, triage, registration, and communication of Telemedicine consult requests for the Health System. This job uses problem-solving and customer service skills to determine the best course of action for the patient by working closely with the practitioner to ensure effective and efficient treatment of every patient. Responsibilities

  • Effectively uses the Nursing Process in the delivery of patient care through data gathering, planning, and evaluation to ensure the patient and provider are prepared for the Telemedicine visit.

  • Completes appropriate documentation for all Telemedicine encounters, including registration if necessary.

,

  • Coordinates and maintains data registry for each Telemedicine program.

,

  • Provides excellent patient service through effective communication both over the phone and through video.

,

  • Maintains proficiency in computer systems and databases for the purpose of excellent patient care, accurate data collection, monitoring, and evaluation.

,

  • Maintains professional and technical knowledge and required licensure and certifications through continuous education and professional development.

,

  • Adapts behavior to the specific patient population, including respect for privacy and communication style.

  • Performs other related duties as required.

Requirements

  • Licensed Practical Nursing diploma.

,

  • Current state licensure from an accredited School of Practical Nursing.

,

  • Current Basic Life Support (BLS) certification from the American Heart Association.

Nice-to-haves

  • 3 years hospital based experience in the delivery of patient care as an LPN.

,

  • Experience in Call Center, Emergency Department, Triage, or Telehealth setting.
