LPN Care Coordinator (Hybrid/Remote) - Primary Care- Lake Terrace

The successful candidate will be responsible for overseeing the disease registry database, conducting pre-visit chart reviews, coordinating care across the continuum, and participating in quality improvement activities. Responsibilities

• Assists in data collection and implementation of healthcare services.

Requirements

• Graduate of an accredited school of practical nursing.

• 2 years of ambulatory care experience required.

• Current licensed practical nurse (LPN) license in state of practice required.

• Proficiency in using computers, software, and web-based applications.

• Effective verbal and written communication skills.

• Ability to present information clearly and professionally.

• Highly motivated and eager to learn new skills.

Nice-to-haves
