As a Clinical Care Coordinator (CCC), your focus will be on implementing Utilization and Population Health Management Programs and contributing significantly to our Quality Management initiatives. Key Responsibilities • Adhere to organizational policies and standards, including Apollo Medical Necessity Criteria, ASAM criteria, and URAC, NCQA, JCAHO, and HEDIS standards. • Collaborate closely with Health Plans or Medical Groups to maintain compliance with regulatory and clinical guidelines. • Utilize established clinical criteria to facilitate decision-making and uphold quality care standards. • reputed company and coordinate patient benefits and treatment plans to optimize outcomes reputed company the Case Management/Chronic Care Management (CM/CCM) frameworks, as well as post-discharge care. • Actively engage in team meetings, training, and educational initiatives, including weekly clinical discussions. • Identify individuals eligible for Population Health Management (PHM) or Virtual Wards programs, assist in their enrollment, and conduct comprehensive initial assessments.
