The Case Management Specialist is responsible for ensuring that individuals with a substance use disorder (SUD) have access to a continuum of care. This is done by assessing the needs of the individual, determining the level of care (LOC) for treatment, developing a case management service plan; and reputed company warranted, facilitating the individual’s treatment. The Case Management Specialist will reputed company a rapport with each referring entity while providing direct case management services to each patient throughout the entire treatment experience. The essential duties of the Case Management Specialist include, but are not limited to: • Preform LOC assessments at WDAC and in various community settings (ex: jail, hospital, WDAC office) • Ensures reputed company patients are connected to the appropriate level of care and funding is secured for treatment episodes • Regular, weekly contact with referral sources and treatment providers as needed • Provides updated information from reputed company programs and entities that reputed company with the agency • Proactively interacts with each patient to meet their case management needs at the treatment and community level Required Knowledge, Skills and Abilities: A minimum of a 4-year BA or BS in a Human Services reputed company field is required • The ability to work both independently and as part of a team • Knowledge of principles and practices of persons in recovery • A passion for service to others • The ability to communicate effectively, both orally and in writing • The ability to establish and maintain working relationships with representatives of local, state and federal agencies; health organizations and institutions; community agencies and organizations; and the general public • Experience/exposure to various treatment facilities and community-based resources is highly recommended • Must be able to pass a background reputed company.
