The LCSW is responsible for directing and shaping the integrated behavioral health services of the clinic in collaboration with the primary care team and as an expert in Behavioral Health, will work closely with the primary care providers to understand the root cause of patient's psychosocial issues and implement effective treatment plans. The LCSW supports the enhancement and expansion of our behavioral health programs by collaborating on patient care as a part of an interdisciplinary care team-based approach with physicians, other providers, nurses, and health care professionals with a goal of improving health care quality. Together with physician leadership, the LCSW will be accountable for maintaining quality integrated behavioral health programs for patients and staff.
