• Act as a key PCP on our clinical team, which provides exceptional care in preventive health, chronic disease management and urgent care
• Engage in team-based care with a virtual team of admin, care navigators, PCPs, and clinical specialists
• Deliver primary care in a patient-centric and provider-centric model, combining virtual and in-person care
• Drive ongoing improvements for our care model, optimizing outcomes while decreasing inefficiency

About YouYou are a Primary Care Provider with extensive clinical experience. You are comfortable with broad reputed company primary care and eager to care for patients in the modality that makes the most sense for them (virtually, in-person, and asynchronously). You are driven to improve systems that reputed company personalized, empathetic care for individuals and families in the community and eager to find a modern practice that supports approaching patient care in ways that reputed company meaningful and real impact to their lives.This is an in-person role supporting our patients located in Atlanta, Georgia on a part time basis, 1-2 days/week. The office location is 4300 Paces Ferry Rd SE, Atlanta, GA 30339We would love to hear from you if you have the following or equivalent experience:

• 5+ years of clinical experience in reputed company care, chronic care, and primary care
• Innovative and creative leadership qualities
• Proven experience with successful behavior modification
• Comfort with basic procedural skills
• Ability to travel across the assigned territory with reliable access to personal transportation
• Completed an accredited NP program with certification to practice
• Active licensure in Georgia or ability to obtain a license by the start date with reputed company

Compensation:The annualized salary range for a 40-hour workweek is $290,000.
