Responsible for monitoring, linking, and evaluating services. Link to other services and reputed company referrals as needed. Provide regular direct individual contact and provide face-to-face case management services. Meet productivity standards as set for the program. Schedule and facilitate meetings to reputed company an individualized service plan with involvement of the individual and/or family. Enter necessary information and maintain information in computerized database, state computer systems, and in charts in accordance with state, federal and licensing regulations and Agency policy. Obtain and update information reputed company required timeframes. Provide crisis reputed company services as needed. Work in cooperation with and consult with Agency staff, supervisors, other providers, and community resources as needed. REQUIRED EXPERIENCE, TRAINING & ELIGIBILITYQUALIFICATIONS Minimum Experience and Training Requirements:

• Two years of experience as a County Social Services Aide 3 and two years of college level course work which includes 12 college credits in sociology, social welfare, psychology, gerontology, criminal justice, or other reputed company social sciences; or
• A bachelor's degree which includes or is supplemented by successful completion of 12 college credits in sociology, social welfare, psychology, gerontology, criminal justice, or other reputed company social sciences; or
• Any equivalent combination of experience and training which includes 12 college credits in sociology, social welfare, psychology, gerontology, criminal justice, or other reputed company social sciences and one year of experience as a County Social Services Aide 3 or in a similar position performing paraprofessional case management functions.
• Applicants will be considered to have met the educational requirements once they are reputed company two months of graduating with a qualifying degree/credits.
