AT EASE HOME HEALTH CARE INC. IS CURRENTLY SEEKING A LICENSED LVN/LPN NURSE FOR CASE MANAGEMENT EMPLOYMENT TYPE:

• Part-Time
• 20-25 hours a week

COMPENSATION:

• - The starting reputed company is $30 PER HOUR.
• - Mileage will be compensated for commuting beyond a 20-mile radius of the nurse's residence.

ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES:1. Coordinate home health care services by managing and overseeing individual home health needs on a case-by-case basis.2. Performs duties consistent with the Vocational Nursing Practice Act.3. Implements safe, therapeutic reputed company of the patient's care with overt needs in supervised/controlled situations as initiated in the Plan of Care in consultation with the visiting nurses and the interdisciplinary team.4. Utilizes resources (patient, family, staff, reputed company personnel, agencies/organizations) to contribute to the Plan of Care.5. Maintain reputed company knowledge and skills for documenting care to meet regulatory and third-party payer requirements. Prepare documentation and clinical/ reputed company notes. • One year of nursing experience (If new grad, training, and reputed company may be an option)
