About the position Acentra Health is hiring Clinical Evaluators (PASRR) to join our growing and dynamic team! As a Clinical Evaluator PASRR on our Pre-Admission Screening and Resident Review (PASRR) team, you will play a vital role in ensuring individuals receive the right care at the right time. By conducting face-to-face assessments in Topeka, Kansas, and its surrounding areas, you'll help facilitate appropriate placements and enhance the quality of life for those you serve. This part-time hybrid role offers the perfect balance of flexibility and purpose. Work from the comfort of your home office while also engaging directly with patients in local settings, such as nursing homes and hospitals. Travel locally to perform essential evaluations, with all travel expenses, including mileage and parking, fully reimbursed. Your work will focus on completing clinical assessments for individuals with serious mental illness (SMI), intellectual disabilities (ID), or related conditions (RC) as they transition to nursing home care. These evaluations are crucial in determining the next level of care, improving patient well-being, and supporting better health outcomes.

Responsibilities

  • Conduct accurate screenings, evaluations, and determinations.

  • Develop informed level of care recommendations based on thorough clinical evaluations.

,

  • Conduct assessments in various environments, including nursing homes, hospitals, and community settings.

,

  • Adhere to the state's best standards of mental health practices, intellectual and developmental disabilities, geriatrics, and other conditions requiring nursing care services and supports.

  • Ensure compliance with contractual and regulatory standards while developing level of care recommendations.

,

  • Utilize independent judgment, deep clinical expertise, exceptional communication skills, and problem-solving abilities.

,

  • Foster effective collaboration with a diverse range of healthcare professionals, patients, families, and support networks.

,

  • Leverage technology by utilizing the state's electronic system and proprietary platforms to manage and track assessment activities.

,

  • Stay informed on current literature, regulations, and research pertaining to long-term care, mental illness, medical conditions, and geriatrics.

,

  • Ensure compliance with necessary workflows to maintain quality and efficiency.

,

  • Efficiently prioritize and execute tasks with attention to detail.

Requirements

  • Active, unrestricted licensure in Kansas in a healthcare or related field.

,

  • Minimum of 2 years' experience in assessing and diagnosing individuals with mental/psychiatric disorders.

,

  • At least 1 year of experience working with individuals with intellectual disabilities or related conditions.

,

  • Graduation from an accredited college with a degree relevant to the field.

,

  • Working knowledge of the Preadmission Screening and Resident Review (PASRR) requirements.

,

  • Skilled in assessing and analyzing psychological, emotional, behavioral, and physical functioning.

,

  • Knowledge of psychiatric disorders as outlined by the latest Diagnostic and Statistical Manual.

,

  • Understanding of standards of practice for determining Level of Care.

,

  • Strong familiarity with public mental health and medical programs.

,

  • Demonstrated experience working with the geriatric population.

,

  • Proven ability to formulate clinical diagnoses, considering psychological and behavioral factors.

,

  • Strong communication skills, capable of effective interaction with individuals and groups.

,

  • Skilled in using computer equipment based software and proficiency in Microsoft Office Suites.

,

  • Detail-oriented with the ability to prioritize and execute tasks efficiently.
