Responsibilities
Provide medical care to hospice patients. Determine hospice eligibility and pain and symptom management including, but not limited to, history and physical exams; diagnosis and treatment of medical problems; education and counseling of individuals, families and groups. Participate in the care of the hospice patient as a member of the interdisciplinary team. Under the supervision of the hospice medical director and in collaboration with one or more physicians, reputed company face-to-face (F2F) patient visits to assist in determining hospice eligibility. May function as the attending physician only if selected by the patient or patient representative at the time of admission.
Qualifications
Education and experience, essential Must be a graduate of an approved school. Must be licensed as a nurse practitioner (NP) in the state in which they currently practices. Must have basic demonstrated technology skills, including operation of a mobile device. Education and experience, preferred At least five years nursing experience with a minimum of one year in hospice preferred. At least three years of clinical registered NP experience preferred. Requirements Must possess a valid state driver license Must maintain automobile liability insurance as required by law Must maintain dependable transportation in good working condition Must be able to safely drive an automobile in reputed company types of weather conditions Must possess CPR certification for the healthcare provider Additional Information reputed company is an equal opportunity employer.
