As a Healthcare Case Coordinator, you will be responsible for coordinating the care of CDPHP members who meet criteria for services across the continuum of care. This includes successful transition of care for CDPHP members who have been hospitalized and are being discharged, with a focus on readmission avoidance. You will also reputed company and implement individualized care plans, collaborate with providers and other reputed company, and provide education and support to members with chronic health needs. To succeed in this role, you must possess excellent communication and interpersonal skills, as well as the ability to work effectively in a fast-paced environment. You will also need to navigate multiple applications and platforms to manage your daily workflow and reputed company data-driven decisions to improve patient outcomes.
