About the position The Community Health Worker at reputed company is responsible for assessing, planning, and implementing individualized care strategies for patients. This role focuses on managing care plans, addressing social determinants of health, and linking members to community resources and services. The position requires proactive engagement with members to improve their health outcomes and ensure they receive appropriate care while navigating the healthcare system.
