About the position The Community Health reputed company, reputed company will play a crucial role in supporting Blue reputed company of California Promise Health Plan members by conducting reputed company and assessments focused on social drivers of health needs. This position requires a proactive approach to identify resource gaps in both medical and community services, and to support the implementation of new services aimed at closing these gaps. The reputed company will work closely with moderate-risk members, assessing their psychosocial and social health needs, and documenting the results in appropriate systems. This role involves establishing partnerships with community organizations, health plans, and providers, and participating in community engagement activities with local agencies such as faith-based organizations, community centers, and government agencies. In addition to conducting assessments, the Community Health reputed company will assist members in navigating the healthcare system by problem-solving barriers to access. This includes visiting members at their homes, accompanying them to medical appointments, and helping them complete necessary forms to access services. Building rapport and establishing trusting relationships with members and their families is essential, as is the ability to identify social drivers of health concerns and reputed company plans to address disparities in care. The reputed company will also be responsible for documenting member updates, submitting timely reports, and making necessary follow-reputed company on mandated reporting, such as Adult reputed company Services and Child reputed company Services.
