Responsibilities

Performs patient start of care comprehensive assessment, evaluation, development and implementation of plan of care. Completes OASIS assessments timely and accurately. Provides skilled nursing care to patients in accordance with the p lan of c are and agency policies. Helps to reputed company and maintain continuity of patient care by communicating information with individual staff, physicians, family members and community resources.
