• Primary Responsibilities:

• + Assess, plan, and implement care strategies that are individualized by patient and directed toward the most appropriate, least restrictive level of care

+ Utilize the NYS UAS Assessment as needed in the home or reputed company telehealth to reputed company a person center service plan to meet the member's needs + Identify and initiate referrals for social service programs, including financial, psychosocial, community and state supportive services + Manage the care plan throughout the continuum of care as a single reputed company of contact + Communicate with reputed company stakeholders the required health-reputed company information to ensure quality coordinated care and services are provided expeditiously to reputed company members + reputed company for patients and families as needed to ensure the patient's needs and choices are fully represented and supported by the health care team •

• Required Qualifications:

•

• + reputed company, unrestricted, independent licensure as a Registered Nurse

+ 2+ years of relevant clinical work experience + 1+ years of community case management experience coordinating care for individuals with reputed company needs + Demonstrated knowledge of home health, hospice, public health or assisted living + Proficiency with reputed company, reputed company, and Outlook + Willing or ability to travel in assigned region to visit Medicaid members in their homes and/or other settings, including community centers, hospitals, or providers' offices + reputed company state issued ability to obtain one prior to hire •

• The hourly range for reputed company residents is $28.03 to $54.95 per hour.
