Pharmacy Technician, Site of Care Coordinator Work From Home

Position Summary This position is work from home and open nationwide. Responsible for providing care coordination and health care navigation assistance to members on specific specialty infused medications. Primary Duties And Responsibilities • Interacts with patients, medical office staff, and health plan providers through both inbound/ outbound calls, explaining benefits regarding site of care and facilitating changes to ensure a smooth transition in patient care. • Provides care coordination and health care navigation assistance to members. • Facilitates member understanding of benefit change as it relates to specialty medications and works with physicians and their agents to transition to alternate site of care as applicable. • Communicates with member’s PCP, specialist, or the clinical team when needed to address gaps in care. Improves compliance with treatment plans or insurance benefit design. • Works with Specialty Pharmacy to ensure member transition without interruption in therapy. • Performs other duties and responsibilities as assigned. Operating hours: 8:00am - 8:00pm EST, Monday - Friday Schedule: four 10 hour shifts, 8:00am - 7:00pm EST and 9:00am - 8:00pm EST. (shifts rotate each week, colleagues work 4 days with 1 day off during the week) 1 hour lunch break. Required Qualifications • Valid Pharmacy Technician License per state requirements • National Certification - PTCB or ExCPT • Minimum two years of experience as a pharmacy technician. • Previous telephonic experience in healthcare; prior work experience in infusion, medical health office, or specialty pharmacy setting preferred. Preferred Qualifications • Excellent verbal and written communications and listening skills. • Proficiency with Microsoft Office, including Word, Excel, and Outlook. • Ability to perform independently; be self-directed. • Previous work from home experience. Education • High School Diploma Pay Range The Typical Pay Range For This Role Is $17.00 - $25.65 This pay range represents the base hourly rate or base annual full-time salary for all positions in the job grade within which this position falls. The actual base salary offer will depend on a variety of factors including experience, education, geography and other relevant factors. This position is eligible for a CVS Health bonus, commission or short-term incentive program in addition to the base pay range listed above.
