Key Responsibilities
Monitor patient data and alerts from remote monitoring devices, ensuring prompt follow-up with patients and healthcare providers. Engage with patients to develop monthly care plans, provide education on managing chronic conditions, and address any questions or concerns. Maintain accurate and up-to-date patient records, adhering to healthcare regulations and privacy standards. Collaborate with healthcare providers to tailor care plans to meet each patient's unique needs.
Qualifications
Experience in chronic care management or remote patient monitoring is a plus. Exceptional organizational and multitasking abilities. Strong communication and interpersonal skills.
