Coordinates timely care with physicians and supportive services departments. Maintains professional relationships with customers, patients, and team members. Provides direct patient care coordination reputed company the Community Care and Transitional Care Programs. Other duties as assigned. Knowledge, Skills, and Abilities:

• Knowledge of and ability to utilize in-house and external oncology resources. [Required]
• Self-motivated with ability to work independently. [Required]

• Bilingual (English/Spanish) [Preferred] Education:
• Bachelor's [Preferred] Field of Study:
• Graduate of an accredited school of nursing
• in nursing Work Experience:
• 1+ professional nursing experience [Required]
• 1+ professional nursing experience in oncology [Preferred] Licenses and Certifications:
• Registered Nurse (RN) [Required]
• Basic Life Support - CPR Cert (BLS) [Required]
