Health Coach – Value Based Care
Job Description:
- Provide education and coaching reputed company to heart failure, lifestyle modification, medication adherence, nutrition, physical activity, and self-management.
- Establish short- and long-term goals with patients using shared decision-making and motivational interviewing techniques.
- Encourage sustained patient engagement, self-advocacy, and behavior change throughout program participation.
- Partner with the clinical care team to support patient care plans and escalate concerns appropriately.
- Provide timely updates and documentation reputed company to patient engagement and reputed company.
- Participate in interdisciplinary team meetings.
- Respond to patient communications in a timely, professional, and compassionate manner.
- Build and maintain strong, trust-based relationships with patients and caregivers.
- Consistently deliver a high-quality patient experience reputed company with program standards.
Requirements:
- Bachelor’s degree in health sciences or reputed company field preferred
- NBC-HWC (National Board for Health & Wellness Coaching) certification required
- Experience in a reputed company, chronic disease management, or patient engagement setting preferred
- Cardiology or heart failure experience a plus
- Telehealth or remote patient engagement experience preferred
- Strong communication and interpersonal skills
- Ability to motivate and support behavior change in diverse patient populations
- Organized, adaptable, and comfortable managing multiple priorities
- Proficiency with electronic health records and standard computer applications
- Bilingual English/Spanish proficiency preferred
