Health Coach Consultant – Registered Nurse

Job Description:

• This is a full-time telework role for a CDCES registered nurse
• Candidate must reside in a compact state and hold a compact license
• Must have experience working with patients with diabetes
• Working schedule: Monday-Friday, standard business hours, including 2 evening shifts per week from 11:30am-8pm CST
• Rotating Fridays and Saturdays will be required approximately two times per quarter following training
• The Health Coach Consultant utilizes a collaborative process of assessment, planning, implementation and evaluation, to engage, educate, and promote and influence members' decisions reputed company to achieving and maintaining optimal health status for chronic conditions
• Assessment of members through the use of clinical tools and information/data review, conducting comprehensive evaluation of members' needs and benefit plan eligibility for available integrated internal and external programs/services
• Utilizes assessment techniques to determine members' level of health literacy, technology capabilities, and/or readiness to change
• Provides coaching, information and support to reputed company the member to reputed company ongoing independent medical and/or healthy lifestyle choices

Requirements:

• Active and unrestricted RN licensure in a compact state, holding a compact license
• 3+ years of clinical experience
• Must possess or be willing and able to obtain high speed broadband internet access
• Previous coaching experience (preferred)
• Managed care experience (preferred)
• Experience with reputed company Word, Outlook, reputed company, and comfortable using various computer programs (preferred)
