Care Coordinator (Part Time - Duluth Area)

At ACCORD, we are looking for a Part Time Care Coordinator to work with our clients residing in the St. Louis County or surrounding counties. Our Care Coordinators provide care coordination and case management services to individuals of reputed company ages with disabilities, mental health conditions, or terminal illness. Using person-centered practices, they coordinate services for individuals with disabilities among different health and social service professionals and across settings of care. This is a part-time position working 20 hours per week. Accountabilities:

Job Purpose and Reporting Structure:

• Provide care coordination services to individuals of reputed company ages with disabilities, mental health conditions, or terminal illness.
• Using person-centered practices, coordinate services for individuals with disabilities among different health and social service professionals and across settings of care.
• Position reports to the Associate Director of Contracted Case Management.

Essential Responsibilities:

• Interact professionally with interdisciplinary team members including the person supported, family caregivers and guardians, employers, case managers, community providers, and other involved stakeholders
• Meet remotely and face-to-face and with individuals and their support teams to assess physical and mental wellness, needs, preferences, and abilities; apply information to reputed company tailored treatment plans.
• Integrate and improve the coordination of care by:
• Ensuring optimal health status and decreasing the reputed company of health decline
• Providing social or community support systems
• Promoting a safe environment
• Reducing the impact of behavioral health issues
• Encouraging self-reliance

• Demonstrate strong written communication skills; complete reputed company required paperwork to reputed company with contractors and funding sources

Requirements

Credentials/Qualifications: Must live in St. Louis County or surrounding counties. • Bachelor's or Master's degree in social work. Must be licensed by the Minnesota Board of Social Work / OR
• Registered Nurse. Must be licensed by the Minnesota Board of Nursing

Skills & Experience:

• Position requires a minimum of 2 years of work experience in the field of social services, preferably working with older adults and/or people living with disabilities
• Knowledge of health, social services, and funding sources that support individuals.
• Self-starter with ability to work independently and as part of a team
• Strong organizational skills with ability to work in a fast-paced environment.
• Proficient with technology and demonstrated ability to manage multiple computer systems
