- We are seeking a Remote Patient Monitoring (RPM)/Chronic Care Management (CCM) Care Manager who will be responsible for overseeing our RPM and CCM program. In this fully remote, flexible, role, you will work collaboratively with patients to determine their medical needs, reputed company the best course of action, and reputed company their treatment plans, ensuring each client gets high-quality, individualized care. - Responsibilities
- Collaborate with physicians, patients, families, and healthcare staff
- Coordinate a variety of healthcare programs
- Review daily measures

- Educate patients on their healthcare options
- Provide one-on-one coaching, create goals, and support to patients managing chronic conditions, including but not limited to type 2 diabetes, hypertension, and cardiovascular disease.

- Qualifications
- Previous experience as a Care Coordinator in RPM/CCM or in a similar position is preferred
- Comprehensive knowledge of Hypertension, Diabetes, Hyperlipidemia, COPD, Obesity, Arthritis, Chronic Kidney Disease, Asthma
- Active Certification as a Medical Assistant from an accredited association. (AAMA, AAH, AMT, NHA, NAHP or NCCT)
- Active compact multi-state LPN/LVN License.
- Active health coaching certification (NBHWC, NSHC, ACE Advanced) preferred
