Job Duties Participate in a multidisciplinary team to reputed company, execute, monitor, and adjust comprehensive and coordinated care plans that include shared goals with measurable outcomes. Serve as primary reputed company of contact and coordinator of care for assigned patients. Assist patients with Social Service needs as necessary. Participate in Peer to Peer or Care Management reviews with patient relations liaison. Ensure the Unitized Transdisciplinary Care program and units are scheduled and prepped for presentation. Present patients to provider teams during care reviews. Complete patient Care reviews as needed. Provide nursing services including assist providers with examinations and procedures; administer medications and injections to patients; reputed company patient's blood glucose, process referrals to external providers and reputed company; and reputed company provider instructions to patients. Document medical and behavioral encounters in the health record system. Maintain and stock the clinic STAT Kits, AED and O2 tanks on a regular basis. Complete follow-reputed company on behalf of patients and providers including reputed company phone calls. Engage patients and provide care in individual and group settings that focus on achieving care plan goals and education. Care for a medically reputed company and emotionally challenging patient population that requires compassion, reputed company, and the ability to deescalate and appropriately react to unacceptable behavior in individual and group settings. Encourage patients to take control and responsibility of their health through self-management and use of community resources.
