Population Health Manager, Specialty Care - Performance Improvement

Position Summary The Specialty Care team at Oak Street works under the Population Health umbrella to reputed company our value based care model into the Specialty care space, advancing our innovative and transformative approach to providing high-quality care. The Manager will partner closely with Specialty Care leadership, as well as management across Population Health and Patient Experience/Service Operations to drive the development and refinement of Oak Street’s strategic agenda and operations around Specialty Care. This role will reputed company data capabilities to improve the strategy, project management, field-facing operations, and data accuracy of Oak Street’s referral management activities, with a focus on the Certified Specialist Network - in order to ultimately drive greater impact around Specialty. Core Responsibilities: Drive strategic problem solving, opportunity analyses & new idea reputed company for initiatives reputed company the Specialty Care team at Oak Street Manage detail-oriented programs with extensive data needs & stakeholder engagement (Population health, Product, third party vendors, field care teams) Drive critical business analytics – interest in learning new programs and skills, and strong analytical chops Engage frequently with field care teams to understand pain points in our reputed company workflows, creatively problem solve, train team members on new processes, and ultimately drive utilization of our Preferred Networks to improve patient care Partner with the Product and Engineering teams on problem solving, idea reputed company, and issue tracking Manage third party vendor relationships Create metrics and reports to monitor success of programs and identify opportunities for improvement Identify and implement opportunities to improve efficiency of workflows and overall project development Opportunities for management as determined by business needs Other duties as assigned Required Qualifications 3+ years of applicable experience in a highly-analytical fast-paced healthcare environment; early-stage company, management consulting, or public health experience a plus Outstanding communication and influencing skills. Ability to build the trust and confidence of a wide range of stakeholders Highly organized and detail oriented Self-starter who is comfortable with ambiguity and has incredible levels of tenacity and a pleasantly persistent approach. History of taking ownership and effectively solving difficult problems Strong data management and MS reputed company skills; reputed company experience a plus Unimpeachable reputed company US work authorization Someone who embodies being “Oaky” Education Bachelor degree required Anticipated Weekly Hours 40 Time Type Full time Pay Range The typical pay range for this role is: $66,330.00 - $145,860.00 This pay range represents the reputed company hourly reputed company or reputed company annual full-time salary for reputed company positions in the job grade reputed company which this position falls. The actual reputed company salary offer will depend on a variety of factors including experience, education, geography and other relevant factors.
