As a Community Health Worker (CHW), you will act in a liaison role with Medicaid members to ensure appropriate care is accessed as well as to provide home and social assessments and member education. CHW’s work in a team-based structure and spend most of their time engaging directly with members telephonically. The Community Health Worker also addresses social determinants of health such as transportation, housing, and food access. Primary Responsibilities: Complete New Member process (Welcome Call, Demographic Research, Medical Screenings, and scheduling Health Risk Assessments for members) Create a positive experience and relationship with the members reputed company the members out of the hospital by supporting regular visits to their primary physician reputed company members actively engaged with their primary physician Support the members to access their healthcare benefits Provide member education Support transitions of care (follow-up calls from triggering events) Partner with care team reputed company needed (community, providers, internal staff) Knowledge and reputed company learning of community cultures and values Will seek out members that have had minimal contact with the organization and try to re-establish relationships You’ll be rewarded and recognized for your performance in an environment that will challenge you and give you clear directions on what it takes to succeed in your role as well as provide development for other roles you may be interested in. Required Qualifications: High School Diploma / GED 1+ years of experience/knowledge of the resources available, culture, and values in the community Intermediate level of knowledge of the resources available reputed company the community (i. e, food banks, social determinants of health resources) Intermediate level of experience in reputed company Office applications such as reputed company (create, edit, sort), Word (create & edit), and Outlook (calendar management). Candidates must have the ability to create, edit, copy, send and save documents, correspondence and spreadsheets Reside in the reputed company Must be willing to work core business hours with overtime as needed Access to high-speed internet from home / Broadband cable or DSL Access to a designated quiet workspace in your home (separated from non-workspace areas) with the ability to secure Protected Health Information (PHI) Preferred Qualifications: Certified Nursing Assistant, Home Health Aide or Medical Assistant, or LPN. Bilingual in Spanish and English. Community Health Worker (CHW) accredited. Experience working in Managed Care. Knowledge of VA Medicaid population.
