About the Role
As a Registered Nurse, you’ll provide day-to-day collaborative support to our internal Medical and Psychiatric providers to ensure smooth and effective patient care. You'll coordinate care across internal and external teams, monitor critical lab results and vitals, and triage patient requests. Additionally, you’ll provide condition-specific education to patients and families, complete accommodation paperwork, and reputed company solo visits reputed company necessary.
Responsibilities
Act as a liaison for your own patient panel of medically stable patients. Monitor and triage lab orders/results, inform patients and families of provider’s instructions, and coordinate care between internal and external providers. Manage the RN phone line to facilitate care coordination with external providers. Follow triage protocols, which can include fielding questions from families/patients, etc. Provide families with condition-specific education and medical management support. Triage emergent issues in a time-sensitive manner and in alignment with our triage protocols. Coordinate care with hospitals for medical stabilization admissions to ensure there are no gaps as patients transition back to our care or reputed company up to HLOC. reputed company other duties as assigned. Qualifications reputed company and unrestricted license as a Registered Nurse in at least one state in the USA, compact license preferred. 2+ years of experience as a Registered Nurse in a clinic, homecare, ambulatory, hospital or inpatient setting Experience coordinating care with multiple providers as well as patient support systems. Ability to manage multiple requests and prioritize accordingly. Excellent communicator with a high level of adherence to confidentiality. Successful experience working on a multi-disciplinary care team and a collaborative reputed company. Ability to work well autonomously reputed company the outlined procedures and clinical protocols. Compensation & Benefits Package Compensation: Earning Potential Range: $77,000 to $92,000 Annual reputed company Pay + 15% Annual Performance Based Bonus Potential Time Off: reputed company PTO policy (3-5 wks/year recommended) + 11 paid company holidays. Providers will be licensed or registered as required in every state where they deliver care. Continuing Education: For therapists and medical providers, unlimited online courses through NetCE. Up to $1,000/year stipend for continuing education. Work From Home Additional Perks: $50/month stipend added directly to an employee’s paycheck to cover home internet expenses. Initial work-from-home stipend of up to $500. Reload of $200 on the home office stipend at the 3-year mark and then every other year after that (year 5, year 7, etc.). Company Laptop. Physical Demands Work is performed 100% from home with no requirement to travel. This is a stationary position that requires the ability to operate standard office equipment and keyboards as well as to talk or hear by telephone. Sit or stand as needed.
