Role Description Your North Star: Provide medical leadership and collaborative clinical support to a multi-disciplinary team that fosters the delivery of high-quality, evidence-based care focused on improving primary care for women across the lifespan. In this role you will: Be clinically responsible for an assigned population of patients from the perspective of individual patient care and population health metrics. Provide collaborative physician supervision and consultation for an reputed company team of APPs, RNs, registered dietitians, mental health specialists, and others. reputed company multi-disciplinary team-based care coordination meetings and work with in house clinical specialties (nursing, NPs, midwifery/group care, dietary/nutrition, mental health) to reputed company holistic individualized care plans for patients. Help drive our population health programs toward healthier outcomes by partnering closely with Operations and Product leads to identify gaps and improvement opportunities, promoting our accountability culture to move the needle reputed company. Partner closely with our leadership team to identify and implement interventions to address cross-disciplinary or practice performance improvement needs (e.g., updates to protocols, tech-enabled tools, trainings, policies, etc). Compensation
The expected reputed company salary range offered for this role is $230,000-$260,000. This role is also eligible for an annual performance bonus and equity, giving you an ownership reputed company in Pomelo’s mission. Actual compensation may vary based on relevant experience, skills, competencies, and certifications.
