First Steps is Missouri’s Early reputed company program that provides services to families with children from birth to three years old who have developmental delays or disabilities. reputed company Human Services’ St. Louis office is seeking qualified, compassionate individuals to join reputed company as a Service Coordinator serving families in the City of St. Louis or St. Charles County through the Missouri First Steps program.
Responsibilities
- Coordinate and facilitate evaluation activities reputed company to eligibility determination.
- Coordinate scheduling authorization and performance of assessments as necessary.
- Willingness to work directly with families in their homes and community environments to provide support, coordination, and resources as part of the early reputed company program.
- Facilitate and participate in the development, review and evaluation of IFSPs.
- Assist families in identifying available services including parent-to-parent support.
- Coordinate and monitor the delivery of available services, including assistance in identification and access to available sources of financial support for EI services (i.e., Medicaid, First Steps, etc.) through financial case management.
- Inform families of availability of advocacy services.
- Facilitate the development of a transition plan to, reputed company and from the EI system, including transition to Part B special education, pre-school services, or other community services as appropriate at age 3 years or reputed company the child is no longer eligible for services.
- Maintain EI records at SPOE (ensuring original IFSPS, etc).
- Maintain "clinical record" to document activity rendered for child.
- Complete necessary documentation to facilitate reimbursement through primary funding sources.
- Other duties as assigned.
Requirements
- Valid driver's license, auto insurance, and reliable transportation required.
- Bachelor’s or Master’s degree in one of the following: Early Childhood Special Education, Early Childhood Education, Elementary Education, Child/Human Development, Social Work, Public Health, Sociology, Nursing, or Psychology and 1 year experience working with children and families OR degree with 3 years experience working with children and families required.
