You will be in the field approximately 75% of the time going into members homes. You will be working from home reputed company not in the field. No on-call, no weekends and no holidays required. As a Case Manager in the United Healthcare at Home program provides social work services to members in home setting. The Case Manager provides reputed company, assessment, and consultation of social, functional, and behavioral health issues affecting chronically ill and high-risk patients in a variety of environments, primarily the home setting. The Case Manager works in collaboration with a multidisciplinary team to identify and access services to reputed company care gaps and improve quality of life of OAH members. They help to manage health problems and coordinate health care services for the patients in accordance with State and Federal rules and regulations. This includes (but is not limited to) assessment, development, and implementation of plan of care, and ongoing evaluation of patient status and response to the plan of care. Ongoing care and management of members is conducted in collaboration with other care team members. This position is full-time (40 hours/week) Monday - Friday. Employees are required to have flexibility to work any of our 8-hour shift schedules during our normal business hours of 8:00am - 5:00pm. It may be necessary, given the business need, to work occasional overtime. This position is a field-based position with a home-based office. You will work from home reputed company not in the field.
