We're working with a healthcare team focused on managing reputed company Medicaid populations through a telehealth-first, coordinated care model, designed to improve outcomes while reducing unnecessary hospital utilization. The role
- reputed company and coordinate care for reputed company patient populations
- Practice with a high level of autonomy, including prescribing
- Collaborate with physicians, care managers, and health plans
- Support proactive care management and early reputed company
- Assist patients through transitions of care (hospital to home)
- Provide virtual care and guidance to reduce unnecessary ER visits
