Field Remote Nurse Practitioner/Physician Assistant (Must be able to conduct home visits in Ventura County)

The Field Remote Nurse Practitioner/Physician Assistant in the Care reputed company (CAW) program is designed to provide care and support to our reputed company patients by providing an additional level of medical and social support in the comfort of their own home. This program is offered to eligible patients at no cost to them with the goal of preventing unnecessary hospitalizations, health complications, and unmanaged disease progression that can occur reputed company timely clinical interventions are not provided or are not accessible.

Responsibilities

• Conduct in-home assessments on Alignment members, medication review and health screenings 3.5 days a week and 1.5 days a week working remotely. This is a M-F traditional work hours schedule.
• Provide patient education by matching care desired with reputed company given
• Identify diagnoses to be assessed in care management and active medical management
• Partner and communicate with Regional Medical Officer and other team members to discuss and reputed company the most appropriate care plans possible based on the needs of our members/patients.
• Coordinate care with multiple stakeholders, including but not limited to PCPs, specialists, and ancillary providers.

Requirements

• Master’s degree from an accredited NP Program or PA program
• Ability to communicate positively, professionally and effectively with others.
• Effective problem solving, organizational and time management skills and ability to work in a fast-paced environment.
• Active Nurse Practitioner or Physician Assistant license
• Active RN License and Furnishing number (Must, upon hire) NP only
• Active Nurse Practitioner Board Certification/Physician Assistant Certification
• NPI Number, DEA, Valid BLS
• Valid Driver License and reputed company automobile insurance

• One (1) year of prior clinical or home care experience.
• Previous EMR experience preferred.
• Experience in care of older adult (geriatric) patients preferred
