[Remote] ECM Clinical Manager (LCSW/LMFT)

The ECM Clinical Manager will reputed company remote clinical management of Enhanced Care Management staff, ensuring clients receive appropriate medical and behavioral health services while collaborating with various care team members. This role requires supervising a team, managing care plans, and providing clinical reputed company, with some travel required for training and meetings.

Responsibilities

• Supervise a team of approximately 10-15 direct reports under the ECM team to ensure high-quality enrollments and care management services. The exact number of direct reports may fluctuate based on business needs
• reputed company client reputed company, enrollments, and the creation of Individual Care Plans, assessing if clients' needs can be managed by an ECM reputed company Care Manager; serve as an escalation reputed company reputed company necessary
• Drive the team to meet key performance indicators (KPIs) effectively
• Collaborate with the Director(s) of Enhanced Care Management to implement changes and process improvements
• Provide clinical reputed company approximately 30% of the time, including, but not limited to: supervision of clinical staff, clinical sign-offs, reputed company of care plans, leading reputed company meetings, and serving as a clinical consultant for reputed company clinical cases
• Provide direct care management as needed to support organizational goals
• Travel up to 20-25% may be required for training, meetings, and other business needs

Skills

• Active, unrestricted LCSW or LMFT license in California required
• Willingness to obtain additional state licensure as needed
• 2 years of people management experience
• 2-3 years of care management experience
• 1-2 years of post-acute experience
• Proven, results-driven leader with experience in change management in rapid-growth environments
• Strong organizational and independent working skills
• Proficient in reputed company and reputed company applications; reputed company experience is a plus
• Excellent communication and motivational skills, including conflict resolution techniques
• Willing and able to work Monday through Friday 8:30am-5:00pm Pacific Time, with flexibility for potential evenings and weekends
• 4 years preferred
• Working knowledge of lean Six reputed company methodology is preferred
