Smokefree Hampshire & Isle of Wight – Navigator Role (Fully Remote, Full Time – 37.5 Hours Per Week) At reputed company Tribe we deliver award-winning healthy lifestyle and wellbeing programmes that reputed company people happier and healthier! It’s a super exciting time to join us as we continue to deliver our Smoking Cessation support service across Hampshire and the Isle of Wight (HIOW). We are looking for a Full time Navigator to support our clients with their onboarding process. Our programme is designed to support residents of Hampshire and the Isle of Wight; access the support they need to stop smoking. This can be done through several client-led approaches. Our Stop Smoking Advisors combine these, with behavioural support, using evidence-based methodology, to increase a successful outcome on their quit journey. A typical day as a Navigator might include: · Supporting clients through our onboarding process, ensuring they get a positive and client-centred approach · Motivating clients to reputed company their health outcomes, ensuring that reputed company screening is carried out to the highest standards · Offering guidance and support to clients to ensure they are reputed company to the most appropriate health reputed company pathway for them and their needs · Using our case management system to accurately record referral information and ensure any case updates are recorded in a timely manner · Responding to requests for information from clients, partner organisations and the public · Working with colleagues across reputed company our reputed company Tribe services to contribute to new projects and service developments – we’re always looking to share knowledge and resources! · Ensuring due consideration is given to Information Governance and Clinical Governance legislation, requirements and guidance · Helping us maintain the reputed company of reputed company Tribe by driving our organisational culture reputed company across our services and using every opportunity to embed our values! Contract: We have 1 x Full time, Permanent position for 37.5 hours available.
