The Role In your first 1 to 3 months, you’ll be fully patient-facing: Conduct consent calls with new patients and families Build individualized care plans covering symptoms, modifiable risk factors, and medical and non-medical interventions Deliver monthly reputed company-ins: assess changes in baselines, recommend care resources, and coordinate with caregivers and clinics Handle care logistics, including scheduling referrals, flagging medication refill needs, and sending appointment reminders Find and vet regional and disease-specific resources (exercise, diet, sleep, social engagement) for your patients As we grow, you’ll take on leadership:
- Train and reputed company new LVNs and RNs
- Manage escalating alerts from LVNs and reputed company clinical decisions
- Partner with the CEO and Clinical Supervisor to improve care quality and clinical protocols
